"Personal Information" means information that can be used alone or in combination with other information to identify you. Registered Users provide Personal Information during registration, credit card payment, and when entering, saving or submitting health data to receive a purchased prediction test service. In addition, a user, whether registered or not, is submitting Personal Information when submitting his/her email address to sign up for Univfy's newsletters (see Newsletter). A "Registered User" means a person who registers on Univfy's website and interacts directly with Univfy independent of a healthcare provider.
Univfy personnel are trained and follow strict processes that restrict access to Personal Information in connection with the performance of customer support and quality control functions. As much as possible, our quality control functions are conducted with anonymized data or system-generated data using Aggregated User Information (as defined herein) rather than Personal Information.
Univfy will NOT sell, lease, or rent your Personal Information without your explicit consent.
All Registered Users must have an active account and agree to the terms of the Registered User Agreement, which is located at http://www.univfy.com. At the time you create an account, we will ask you for the following information: name, email, phone number, address. To protect the security of your account, you will be prompted to create a user ID and password and provide answers to security questions. This information is referred to herein as "Registration Information."
Registration Information is used to manage your account, authenticate a Registered User, purchase selected Services, provide notifications about website maintenance and updates, and also allows Univfy to communicate with you (for example, regarding Univfy marketing and promotional events and invitations to participate in surveys).
In the event you elect to purchase a Service, you will be prompted to provide Payment Information. "Payment Information" means the information that is collected by Univfy from you in the payment module, whether you pay for the Service in full, at a discount, with a credit or debit card, with a gift card, with some other form of payment, or a promotional code. Payment Information is transmitted directly to CyberSource/Wells Fargo, Univfy's payment gateway, in a secure fashion with SSL encryption, and is not stored on Univfy's server. If Univfy needs to retrieve Payment Information to serve you, then we may need to obtain your Payment Information from CyberSource/Wells Fargo.
Your Payment Information is only used for the specific purpose of collecting payment for a purchased Service. You must be a Registered User to purchase a Service.
To use Univfy's Services, you will need to provide information about yourself and your health. For example, date of birth, age, height, weight, smoking status, ethnicity, family history, past or current fertility treatments and results, clinical laboratory test results such as blood hormonal profile, ultrasound results such as measurements of structures within your reproductive organs like ovaries and uterus, and data about your reproductive partner that may specifically affect your fertility profile. Univfy may collect such Self-Reported information from you online (surveys, input forms, other features online, etc.) or offline (paper survey via mail, telephone survey, mobile applications, texting, etc.). You may contact Univfy's Customer Support to provide Self-Reported Information if you experience technical difficulties.
A "Web Beacon" is a transparent graphic image that is loaded by a user's web browser when it accesses a website and that records a user's visit to a particular web page. Univfy may use Web Beacons to analyze the traffic patterns on its website, such as the frequency with which users visit various parts of its website, and to measure the effectiveness of such website. Web Beacons may be used in association with Cookies to understand how users of the Univfy website interact with the pages and content on the pages of such website. Univfy may also use Web Beacons in HTML emails that it sends to Registered Users who have agreed to receive email from us, to determine whether recipients have opened those emails and/or clicked on links in those emails. No Personal Information you give us is provided for Web Beacon use, so they cannot personally identify you with that information on our website. There is currently no standard or reliable way of disabling Web Beacons.
The only other information passively collected by Univfy is in the form of "Log Files" which record website activity, including how many "hits" a particular web page is getting, the date and time of each visit to a website, the user's IP (internet protocol) address, the referring IP address or domain (the prior website visited), and the files viewed on a website (including HTML pages, graphics, etc.). Univfy may use Log File information to analyze data in the aggregate to determine the trends and usage of our website entries for our marketing and demographic studies and so we can constantly improve the online services we provide you. We may also use the Log Files to troubleshoot technical concerns. Log Files are used internally only, and are not associated with any particular user, computer or browser.
The only Personal Information that is collected when you sign up for Univfy's newsletter is your email address. This email address is used only to send you our newsletters, and is not used for any other purposes, nor is it matched with any other information that you provide as a Registered User. If you wish to opt out of our newsletter at anytime, you can do so by clicking on "Unsubscribe" on the newsletter that you receive from us.
Aggregated User Information may also be used for research programs managed by an Institutional Review Board ("IRB") if and only if you have agreed to a separate consent approved by the applicable IRB ("Research Consent"). The use of your Aggregated User Information will be governed by such Research Consent. The use of the Aggregated User Information subject to a such Research Consent are generally used for publications of scientific research in a peer-reviewed scientific journal or other medium regulated by the IRB.
Please note that all Univfy personnel are trained and follow strict processes that minimize access to data based on the need to perform customer support and quality control functions. As much as possible, our quality control functions are conducted with the use of Aggregated User Information rather than Personal Information, Self-Reported Information, Service Information or personal Online Behavior Information.
Only Aggregated User Information will be provided to Univfy's research personnel and such personnel will have no access to databases that contain Personal Information or linkers to Personal Information. Customer support personnel will not be involved in Univfy research programs.
Univfy may need to comply with requests to have Aggregated User Information verified by a third party/auditor, or analyzed by research collaborators, which may include commercial or non-profit organizations that conduct or support scientific/medical research or conduct or support the development of drugs or devices to diagnose, predict, or treat health conditions.
You may withdraw your Research Consent status at any time by contacting our Privacy Administrator at firstname.lastname@example.org. Your withdrawal of your Research Consent will be effective thirty (30) days thereafter; provided, however, we cannot withdraw information that has previously been used in research programs managed by an IRB in the form of Aggregated User Information prior to the effective date of your withdrawal. Univfy will not use your Self-Reported Information and Service Information in compiling Aggregated User Information for new research programs managed by an IRB after you provide notice of withdrawal. However, Univfy may still use your anonymized Self-Reported Information, Service Information and online Behavior Information included in Aggregated User Information for all other purposes.
In addition we may disclose Personal Information to service providers, including credit card processors, website hosting partners and other parties who assist us in operating our website, conducting our business, or providing you with Services, so long as those parties agree to keep this information confidential.
Univfy does NOT share any Personal Information, Self-Reported Information or Services Information with your healthcare provider. Univfy may provide general Univfy service and product information to healthcare providers who contact us wanting to learn about our services and products and how to discuss Univfy's test results with their patients.
Univfy is not responsible for Personal Information that you disclose on your own or that you request us to disclose to a third party.
Univfy has implemented numerous security features to prevent the unauthorized release of or access to Personal Information. Your Personal Information is protected behind secured networks and is only accessible by a limited number of persons who have special access rights to such systems, and who are required to keep the information confidential. All connections to and from our website are encrypted using Secure Socket Layer (SSL) technology.
At the same time, you share responsibility for protecting your Personal Information by safeguarding your password and other authentication information you use to access the website. Each Registered User's account should only be accessed by that Registered User.
This website may contain links to other websites that may have online information practices different from ours. We are not responsible for the privacy and security practices of those websites, and we encourage our users to learn about the privacy and security practices of those websites.
If your Registration Information changes, you may correct or update your Registration Information via your Account Settings page.
We give you the opportunity to click "Yes" to opt in to receiving one or both of the following optional communications: (i) Univfy newsletter, (ii) Univfy marketing, and (iii) Univfy research emails. If you have previously opted "Yes" to either or both choices, you can change your notifications settings in your account (go to Account, Settings, Notifications) or you can contact us at email@example.com. To opt out of research consent, please email ResearchAdmin@univfy.com.
In addition, we retain limited Registration Information related to your order history (e.g., name, contact, and transaction data) for accounting and compliance purposes.
Univfy does not offer products for sale or services to children under 18. Our services are available to adults only. We do not knowingly collect Personal Information from or provide any Services to any individual under the age of 18. Univfy will not be liable for any damages that may result from misrepresentation of age by a user of our website.
